PHI THETA KAPPA BOOK SCHOLARSHIP

APPLICATION

 

Return this form to the Phi Theta Kappa Advisor's Office in the Student Success Center.  Applications must be received by 4:30 P.M., five working days prior to the first day of class.  Names of recipients will be posted outside the Phi Theta Kappa office by noon, three working days prior to the first day of class. 

HOW DOES THE SCHOLARSHIP WORK?  The Phi Theta Kappa Book Scholarship is primarily a “book loan” scholarship.  If the student’s application is accepted, the Phi Theta Kappa Scholarship will loan the required book(s) or purchase up to $100.00 worth of books for the student.  The student is to use the specified book(s) for the specified semester and then return the book(s) to the Phi Theta Kappa office by the end of the semester.  A hold will be placed on student transcripts or grades until the book is returned.

 

If the student needs to purchase a consumable book that cannot be reused (such as a lab book or workbook), the Scholarship committee will purchase the book, which the student may keep.

 

WHO IS ELIGIBLE FOR THE SCHOLARSHIP?  A student is eligible for assistance if enrolled at LCC and are an inducted member of the Tau Theta Chapter of Phi Theta Kappa.  The student must also have the Phi Theta Kappa maintenance grade point average of 3.25.  LCC students who are not already receiving financial aid for textbooks and who have participated in the Phi Theta Kappa Book Scholarship Fundraisers will be given first choice over those students who are receiving textbook aid.  High School students are not eligible by Kansas state law.  Students must reapply each semester.  If you have further questions contact Tammy Fuentez at 820-1149 or tammyf@Labette.edu.

Recipients need to come to the Phi Theta Kappa Office to sign an agreement and pick up a voucher or a book.  Students will take vouchers to the Financial Aid Office.  Students will then be directed to the bookstore to pick up their book.  Any voucher or book not picked up within 10 business days of recipient list posting is considered forfeit.
